Perioperative Therapie des kolorektalen Karzinoms

Bei Patienten mit operativer Therapie eines kolorektalen Karzinoms besteht ein erhöhtes Risiko einer venösen Thrombose und einer Wundinfektion.

Die optimale Thromboseprophylaxe in der kolorektalen Chirurgie ist die Kombination von physikalischen Maßnahmen (Kompressionsstrümpfe) und Low-dose unfraktioniertem oder niedermolekularem Heparin (Evidenzlevel: 1++, Empfehlungsgrad: A) [Wille-Jörgensen P 2003].

Die perioperative Antibiotikagabe (single-dose) kann die Rate septischer Komplikationen deutlich vermindern (Evidenzlevel: 1+, Empfehlungsgrad: A) [Baum ML 1981] [Zelenitsky SA 2000].

Patienten sollten daher Antibiotika, die sowohl das anaerobe wie das aerobe Keimspektrum abdecken, innerhalb von 30 Minuten vor Beginn der Operation erhalten.

Die Annahme, dass eine mechanische Darmreinigung vor elektiver Kolorektalchirurgie das Risiko einer Anastomoseninsuffizienz und andere Komplikationen senkt, wird durch die Ergebnisse von drei randomisierten Studien und einen Cochrane-Review [Guenaga KF 2003] nicht unterstützt [Burke P 1994] [Guenaga KF 2003] [Miettinen RP 2000] [Oliveira L 1997] [Platell C 1998] [Santos JC 1994]. Andererseits ist der Gegenbeweis nicht schlüssig erbracht, so dass eine abschliessende Stellungnahme hierzu nicht möglich ist [SIGN 2003].

Die Lage des Stomas sollte immer vorher angezeichnet werden. Alle Patienten, bei denen ein temporäres oder permanentes Stoma vorgesehen ist, sollten präoperativ von einer speziell ausgebildeten Fachkraft (Stoma-Therapie) beraten werden.
